Education, Health and Care Plans

Most children and young people with SEN or disabilities have their needs met by their mainstream school, nursery or college, however some may need more support. The local authority can carry out a statutory assessment of education, health and care needs to decide if it necessary to 'make provision in accordance with an EHC plan'. An EHC plan provides a full description of a child or young person's special educational needs as well as any health and social care need, establishes outcomes based on the child or young person's needs and aspirations and specifies the provision required to meet the child or young person's need and outcomes.

If your child has an EHC plan this should be reviewed at least annually. This is called the annual review. Your child's school or setting will hold an annual review meeting with all of the professionals involved in your child's care. You should be fully involved in this meeting.
